NEW MEMBER APPLICATION

From time to time, members bump into someone that they would like to become new members. The Executive Committee provides the following form that outlays those rules and provides a form to be filled out by the current member recommending one for membership.

TOURNAMENT RULES

Give all of the information called for on the form. The Executive Committee must have this information to act upon the request for membership.

After completion, mail this form to the club Secretary/Treasurer.

The Executive Committee is the ruling committee of MUG. If this proposal is favorably acted on by said committee, the proposed member is submitted to the Active Membership. Two or less negative votes constitute acceptance into the club. Three or four negative votes will be referred to the Executive Committee for their ruling. Five or more negative votes constitute rejection by the club.

The President notifies the new member by letter. The new member is fully accepted upon receipt of an initiation fee of $50.00.
